Add links to squid, and mod_ntlm_winbind.
Add information on how the ntlmssp-client-1 protocol is started. Fix 'example' NT and LM responses to be correctly monotonically increasing bytes, represented in hex. Andrew Bartlett (This used to be commit 50ab0ea6b78880bf7e238bf8e0d4ac67a60d81e8)
