Now that we don't allow the smb.conf to change the modules dir, many
functions that simply load modules or initialise a subsytem that may
load modules no longer need an lp_ctx.

the IPC$ share is only used by the ntvfs backends, and doesn't need to
be created on every load of smb.conf. This fixes a problem with
testparm showing the ipc$ share when it isn't defined in smb.conf.
This also removes the admin$ share, which really shouldn't be on by
default. The admin$ share is used for remote software installation,
and normally exposes the c:\windows directory on a windows
server. That makes no sense on Samba. If for some reason a admin$
share is needed, then the admin can create one as usual. Exposing /tmp
via admin$ by default seems like a bad idea.

That means that the pvfs_acl implementations no longer register
as ntvfs modules (which was wrong)

This new module is based on the vfs_cifs module. The idea is to create a backend
which maps SMB requests to a SMB2 server. This will allow existing
test suites for SMB to be run against our SMB2 client and server code.
It will also help validate our SMB2 client library, probably leading
to some API changes to make it flexible enough

We now use a different system for initializing the modules for a subsystem.
Most subsystems now have an init function that looks something like this:
init_module_fn static_init[] = STATIC_AUTH_MODULES;

I hope to eliminate the other init functions later on (the
init_programname_subsystems; defines).

Both subsystems and modules can now have init functions, which can be
specified in .mk files (INIT_FUNCTION = ...)
The build system will define :
- SUBSYSTEM_init_static_modules that calls the init functions of all statically compiled modules. Failing to load will generate an error which is not fatal
- BINARY_init_subsystems that calls the init functions (if defined) for the subsystems the binary depends on
This removes the hack with the "static bool Initialised = " and the
"lazy_init" functions

ntvfs modules
the idea is that a passthru module can use ntvfs_async_state_push() before
calling ntvfs_next_*() and in the _send function it calls
ntvfs_async_state_pop() and then call the upper layer send_fn itself
- ntvfs_nbench is now fully async
- the ntvfs_map_*() functions and the trans(2) mapping functions are not converted yet

taking a context (so when you pass a NULL pointer you end up with
memory in a top level context). Fixed it by changing the API to take a
context. The context is only used if the pointer you are reallocing is
NULL.

something like:
ntvfs handler = nbench posix
and the nbench pass-thru module will be called before the posix
module. The chaining logic is now much saner, and less racy, with each
level in the chain getting its own private pointer rather than relying
on save/restore logic in the pass-thru module.
The only pass-thru module we have at the moment is the nbench one
(which records all traffic in a nbench compatibe format), but I plan
on soon writing a "unixuid" pass-thru module that will implement the
setegid()/setgroups()/seteuid() logic for standard posix uid
handling. This separation of the posix backend from the uid handling
should simplify the code, and make development easier.
I also modified the nbench module so it can do multiple chaining, so
if you want to you can do:
ntvfs module = nbench nbench posix
and it will save 2 copies of the log file in /tmp. This is really only
useful for testing at the moment until we have more than one pass-thru
module.
